Enterovirussen worden vooral in de acute ziektefase met de ontlasting uitgescheiden, maar kunnen vaak nog weken hierin worden aangetoond.De transmissie van enterovirussen vindt plaats door fecaal-oraal contact van mens tot mens (eventueel besmetting van water of voedsel), waarbij vooral jonge kinderen worden besmet, met een snelle verspreiding binnen families en sociale gemeenschappen.
Het bof-, mazelen-, Epstein-Barr- en varicella-zoster-virus wordt overgedragen door druppelbesmetting. Bij rubella is ook diaplacentaire besmetting mogelijk.Teken, muggen, vliegen en vlooien kunnen door hun beet een virus op de mens overdragen
